to help individuals adopt a healthier lifestyle.
When should you seek Diabetes Education?
There are four critical times to assess, provide, and adjust diabetes self-management education and support:
- At diagnosis
- Annual assessment of education, nutrition, and emotional needs
- When new complicating factors influence self-management
- When transitions in care occur

When should you seek Diabetes Education?
Will my insurance cover this?
- Medicare – Covers up to 10 hours in your first 12 months, then up to 2 hours annually.
- Medicaid – Contact your specific plan to find out by calling the number on the back of your card.
- Private Insurance – Most do! Call the number on the back of your card to determine coverage.
How do I get started?
To begin your diabetes education, contact your healthcare provider and let them know you’d like a referral to our Diabetes Education program. Once we receive your referral, a member of our team will reach out to schedule your education.
